Health Requirements for PK-8 Students

Please see the attached information about PK-8 Health Requirements (physicals, immunizations, eye, dental, etc). The proper forms are attached as well. Try to have these items completed and turned in prior to the first day of school. They must be turned in before October 15th in order for your child to continue to attend school . Please read these forms carefully. Fill out all of the student information and sign where indicated before turning them in to the office.
