2022-2023 Health Requirements for PK-8 Students

Please see the attached information about PK-8 Health Requirements (physicals, immunizations, eye, dental, etc).  The proper forms are attached as well.  Try to have these items completed and turned in at registration or as soon as possible after that.  They must be turned in before October 14th in order for your child to continue to attend school.
